The following disclosures are required by the federal Electronic Signatures in Global and National Commerce Act ("E-Sign Act," 15 U.S.C. § 7001 et seq.).

Requesting a paper copy does not withdraw your consent to receive future Communications electronically.
5How to Withdraw Your Consent

Effect of Withdrawal
If you withdraw your consent, we may not be able to provide Services to you.
